分享預設照顧計劃在香港的發展與醫社合作實踐經驗、闡述港島東聯網在晚期照顧的醫社合作模式、探討如何提升助人專業者處理臨終關懷的「自我能力」，並介紹近期針對認知障礙症患者的社區服務。
To share the development of Advance Care Planning in Hong Kong and the medical-social collaboration (MSC) experience, introduce the HKEC MSC model in end-of-life care, discuss “self-competence” of helping professionals in facing death, facing self as well as introducing the new social service initiatives for persons with dementia.
